1.The subject matter of the contract shall be: Supply of endoprotez and other implants to the hospital warehouse
2.The contract is divided into 9 packages and includes:

The contracting authority indicates that the quantities indicated in the price form are estimated quantities.
In view of the specific nature of the subject-matter of the contract and the impossibility of predicting actual requirements, the contracting authority reserves the right to transfer the goods in kind and quantity, provided that the changes made may not result in the financial obligation set out in the tender and in the contract concluded being exceeded.
The contractor may offer only devices which have been placed on the market in accordance with the provisions of the Medical Devices Act of 20.5.2010 (Journal of Laws 2015, item 876).
The contractor may offer a portfolio from one, the same manufacturer, as part of a package. The contracting authority shall not allow the creation of an endoprotez hybrid from components that have been produced by different manufacturers.
Acting pursuant to Article 93(1) 1 point 1 of the Public Procurement Act of 29.1.2004 (Journal of Laws 2015, item 2164 as amended) The contracting authority cancelled
package 1 procedure as no non-rejected offer was submitted.
