Description

1. The subject-matter of the contract is construction and installation works consisting in the ‘Modernisation of three rooms in the Laboratory Diagnostics Department – the Laboratory of Hematology, the Laboratory of General Analytics and Clinical Chemistry’ at the Ludwik Rydygier Specialist Hospital in Kraków sp. z o.o. located in Kraków, Os. Golden Autumn 1, storey+1, segment C1, on plot No. 3/9, Obr. 008, Nowa Huta. 2. The detailed description of the subject of the contract is contained in this Specification, and in particular the Functional and Utility Program constituting Appendix No. 1 to nin. Specifications, as well as the draft contract constituting Annex No. 3 to the Specifications. 3. Elements of works not included in the offer calculation or not valued, and resulting directly from the tender documentation (Functional and Usable Program, Specification) the Contracting Authority will consider as valued and included in the offer, without the possibility of any claims of the Contractor on this account. In connection with the above, the Contractors are required to carefully check the documentation and conditions prevailing on the investment site. The consequences of any errors in the calculation of the tender prepared by the Contractor shall be borne by the Contractor, who must foresee all circumstances that may affect the price of the contract. It is recommended that the Contractor obtain all information that may be necessary for the preparation of the tender, including, in particular, an on-site inspection of the construction site. 4. The Contracting Authority recommends that the Contractor carry out an initial on-site inspection of the places of work in order to obtain the information that may be necessary to prepare the tender for the conclusion of the contract and performance of the contract. Tenders not containing the full scope of the subject of the contract will be rejected. CONDITIONS FOR PARTICIPATION IN THE PROCEEDINGS AND HOW TO ASSESS THESE CONDITIONS 1. Contractors who meet the conditions set out in Article 57 of the Public Procurement Law and Article 112 para. 2 of the Public Procurement Act, i.e.: 1) are not excluded 2) meet the conditions for participation in the procedure, regarding: a) the Contracting Authority does not set a specific condition for participation in the Procedure. b) the right to conduct a specific business or professional activity, the Contracting Authority does not set a specific condition for participation in the Procedure. c) the economic or financial situation, the Contracting Authority does not set a specific condition for participation in the Procedure. d) technical or professional capacity and. the Contractor shall demonstrate that during the last 5 years, and if the period of activity is shorter - during this period, he has carried out at least 1 construction work in an entity performing medical activity [within the meaning of Article 2 par. 1 point 5) the Act of 15 April 2011 on medical activity], for an amount of at least PLN 400,000.00 (in words: four hundred thousand zlotys 00/100) gross. ii. The Contracting Authority will assess the fulfilment of the condition concerning technical or professional capacity, participation in the procedure on the basis of the documents submitted by the Contractor, referred to in Chapter II. X.1 para. 1 (b) of the Specifications.
