Description

1. The subject of the contract is: ‘Purchase of medical equipment and apparatus necessary for the performance of teaching and research tasks in connection with clinical activities’. 2. The subject of the contract was divided into 11 individual tasks: Task 1. Steam (through) Sterilizer Task 2. ICG Laparoscopic Tower Task 3. Patient transport trolley Task 4. Dermatom Task 5. Tools for maxillofacial surgery Task 6. Laryngological toolkit 1 Task 7. Bipolar Tweezers Task 8. Surgical instruments Task 9. Ultrasound camera Task 10. Laryngological Toolkit 2 Task 11. Delivery of a computer for neuronavigation 3. Detailed description of the subject of the contract, technical and functional parameters specify:  Annex 2 – Assortment and price form,  Annex 3 – Technical requirements – Boundary conditions (BIP). 4. The subject of the contract offered must be:  brand new, unused, and free from physical and legal defects; in so far as it is a medical device, meet the requirements of the Medical Devices Act of 7 April 2022 and Regulation (EU) 2017/745 of the European Parliament and of the Council (MDR) or, in the case of a transitional period under Regulation (EU) 2023/607, Directive 93/42/EEC (MDD), and hold the required declarations of conformity and the CE mark; 5. The Contractor shall deliver the subject of the contract to the Contracting Authority’s premises at its own cost and risk (including the costs of loading, transport, insurance for the duration of transport, bringing in, assembling, commissioning and – as regards Task No 1 – dismantling the existing equipment and adapting the installation/room) in accordance with the detailed requirements set out in Annex 3 to the FTZ. 6. The subject of the order will be handed over on the basis of an unreserved acceptance report signed by both Parties. Together with the equipment/assortment, the Contractor will provide complete documentation in Polish (including operating instructions, warranty cards, technical passports with entries for the first start-up) and will carry out free training of the Contracting Authority's staff on the operation and maintenance of the equipment, to the extent specified in the OPZ. The contracting authority provides for the annulment of the procedure pursuant to Article 257(1) of the Public Procurement Act if the public funds which it intended to allocate to finance all or part of the contract have not been allocated to it.
