The subject-matter of the contract is the provision of medical services in the field of occupational medicine to police officers and employees of the Zachodniopomorskie Province and the Management Board in Szczecin of the Central Bureau of Investigation of the Police, issuing on this basis medical certificates/judgments indicating the absence of, or the existence of contraindications to, service/work in the post occupied, including the performance of:
(1) preventive examinations of police officers;
(2) periodic inspections of police staff;
(3) control tests of police officers and police staff;
(4) pre-screening of candidates to work in the Police;
(5) preliminary examinations of persons assigned to a police traineeship;
(6) testing of drivers, including entitling them to drive emergency vehicles and motor boats (driving boats);
(7) examinations when changing conditions of service/work;
(8) medical examinations following the medical committee’s assessment of the police officer’s ability to serve with restrictions and issuing medical certificates indicating limitations to the job description currently occupied;
(9) ophthalmic examinations and certification by a prophylaxis to recommend the use of visual corrective spectacles when operating display screens;
(10) post-exposure prevention controls for HIV infection;
(11) giving an opinion on the state of health of people selected for preventive, rehabilitation and anti-stress tournaments.

2. The subject of the contract will be carried out at a medical facility located in the territory of the city of Koszalin – for the Provincial Police Headquarters in Szczecin (Polices of the Police Headquarters in Szczecin, the Central Police Headquarters, whose place of service is the following localities), Koszalin Municipal Police Headquarters, Police District Headquarters in: Białogardzie, Drawsko Pomorskie, Kołobrzeg, Łobzie, Sławnie, Świdwinie, Szczecinku, Wałcz.
3. The detailed description of the subject-matter of the contract, the quantities, the scope and the conditions for performance of the subject-matter of the contract are set out in Annex 8 to the ToR entitled Description of the subject-matter of the contract and in the model contract, which forms Annex 7 to the ToR.

Participation in the procedure shall be subject to compliance by the Contractor with the minimum condition laid down by the Contracting Authority, which is that the Contractor must ensure that at least 6 medical specialist doctors are available in the same working day of the week for at least 4 hours per day, together with at least one occupational doctor and at least one nurse, and their availability during the same working day of the week for at least 6 hours per day in the case of an occupational doctor and 8 hours per day in the case of a nurse for the entire duration of the contract.
7. Subcontractors: where the performance of a contract is subcontracted, the contractor must indicate in the tender the proportion of the contract to be entrusted to the subcontractors. In the absence of such a declaration, the Contracting Authority will consider that the contractor will perform the contract without the involvement of a subcontractor.
8. The contracting authority shall provide for complementary contracts in accordance with Article 67. 1 point 6 of the PPA to 50 % of the basic contract.
