Description

1. The subject of the contract is: Delivery of containers for pouring cuttings for histopathological examinations compatible with the ULTRA SAFE system and safe containers with formalin for USK-2 PUM. The description of the subject-matter of the contract and the estimated quantities relating to the products are set out in the unit price form (Annex I). 2 to the SWZ, hereinafter also referred to as FCJ) and the required parameters form. (annex. 2a to SWZ, hereinafter also referred to as FPW) 2. The Contracting Authority requires the offer of a) medical devices (applies to all tasks, all items excluding item 6 in task 1)) – within the meaning of the Medical Devices Act (Journal of Laws 2024.1620, as amended), hereinafter: UoWM), placed on the market and used on the territory of the Republic of Poland in accordance with the applicable regulations. 3. The Ordering Party requires that the shelf life of all offered products is a minimum of 12 months, counting from the date of delivery to the Ordering Party's registered office. 4. The Ordering Party has the right to order a smaller quantity of products, with the exception that not less than 60% of the quantity specified in the unit price form. 5. The contract consists of 2 tasks, a detailed description of which is included in FCJ and FPW 6. The contracting authority shall not allow variants to be submitted.
