The subject of the contract are successive deliveries of disposable tools for biliary procedures along with the creation of a deposit for the Health Care Team in Bolesławiec along with transport and bringing to the Ordering Party's warehouses. The detailed description of the subject of the contract includes the Price Form - Annex No. 2.1-2.4 to the SWZ. Each Contractor may submit only one tender for the entire contract or the task(s) concerned in writing, failing which they shall be null and void. If the Contractor submits more than one tender for the entire contract or for a given task/tasks, all tenders of that Contractor will be rejected by the Contracting Authority. The order consists of 4 tasks. The contracting authority shall allow partial tenders to be submitted. As a partial offer, the Ordering Party will consider the offer submitted for all items in a given task. The contracting authority does not allow the submission of tenders for individual items. The Ordering Party reserves that the offered subject of the contract must meet the requirements of the Act of 7 April 2022 on medical devices (Journal of Laws of 2019, No. Journal of Laws 2022, item 974) and Regulation (EU) 2017/745 of the European Parliament and of the Council of 5 April 2017 on medical devices, amending Directive 2001/83/EC, Regulation (EC) No 178/2002 and Regulation (EC) No 1223/2009 and repealing Council Directives 90/385/EEC and 93/42/EEC.
