1. The subject of the contract is the supply of LED headlamps and medical loupes as part of the project entitled: ‘Support for Outpatient Specialist Care and One-Day Treatment at the St. John Paul II Centre for Oncology of the Lublin Region. Jan from Dukla through the purchase of medical equipment and medical apparatus in order to achieve full functionality and security of full-profile, highly specialized diagnostics and treatment of oncological patients." The order was divided into 2 parts: Part 1 – LED headlamps – 2 sets Part 2 – Medical magnifiers – 7 units 2. As part of the performance of the subject of the contract, the Contractor undertakes to: a) deliver the medical equipment being the subject of the contract together with the documents required in the FTZ to the seat of the Ordering Party; b) assembly of medical equipment in the place indicated by the Ordering Party; c) installation and commissioning of medical equipment and confirmation of the efficiency of the delivered equipment; d) providing, together with medical equipment, instructions for use in the Polish language in electronic and paper versions, warranty cards, technical documentation necessary for the proper operation of the apparatus, rules for providing services by an authorized service provider in the post-warranty period, a list of consumables used in the current operation of the subject of the contract, other documentation required by law, implementation of other conditions specified in the FTZ; e) conducting free training of the Employer's staff in the field of operation and operation of the device carried out at the place of installation of the apparatus, certified by a certificate or training protocol. 3. Environmental aspects were used in this procedure: The delivery of the subject-matter of the contract in packaging made of easily recyclable material or from materials derived from renewable sources or should be covered by a reusable system. Packaging materials shall be manually easily sortable and recyclable and shall consist of one type of material (e.g. cardboard, paper, plastic, textile). (b) Training shall be provided which shall include elements on the adjustment and fine-tuning of the parameters of the medical apparatus related to the use of electricity to optimise energy consumption.
