Description

1. The subject-matter of the contract is the supply of diagnostic and procedure equipment for the performance of intravascular procedures for the purposes of the Interlean Specialist Hospital in the range and the quantities shown in detail on the price forms annexed to the ToR, broken down into 10 tasks.
2. The tender may cover all or selected tasks. The submission of partial tenders for individual tasks is not allowed.
3. The contracting authority shall not allow variants to be submitted.
4. The quantity of the subject-matter of the contract must be regarded as an estimate, preliminary, necessary for the conduct of the procedure. The final quantity of the subject-matter of the contract will depend on the Contracting Authority’s actual needs for the duration of the contract.
5. Each Contractor shall be entitled to submit only one tender. The submission by one Contractor of more than one tender or an alternative tender shall result in the rejection of all tenders submitted by the Contractor concerned.
6. The range offered for the tender (listed in the Price Forms) must comply with the description of the subject-matter of the contract, meet the requirements described in the various tasks and items and must meet the conditions for admission to the market in healthcare establishments in accordance with the applicable legislation.

they will be carried out on a continuous basis through a commission warehouse.
8. Payments for equipment delivered to the commission store will be made after it has been used.
9. The Contracting Authority will provide the Contractor with a breakdown of the equipment used, depending on the intensity of its use, once or at most twice per month on the basis of which the Contractor will issue the invoice.
10. Tenders will be evaluated according to the criteria described in Chapter XVII of the ToR.
11. Delivery period from the time the order was placed – not more than 72 hours.
12. The payment deadline offered from the date of submission of the invoice to the Contracting Authority may not be less than 60 days.
13. The contract will be executed for a period of 12 months starting from the date on which the contract is concluded with the Contractor selected as a result of a public procurement procedure.
14. Subcontracting:
(a) The Contractor may subcontract part of the contract.
(b) The Contractor must indicate in the tender the proportions of the contract it intends to subcontract.
(C) The contracting authority informs that it does not reserve the performance by the contractor of essential parts of the contract in person.
15. The draft contracts which form Annex 1a (relating to the range within the commission warehouse) and Annex 1b (relating to the remaining range) to the ToR are integral parts of this documentation and the provisions contained therein are regarded as conditions for the award of the contract.
