Description

1. The subject-matter of the contract is the purchase and successive supply of medicinal products used under the medicines programmes listed in three lots for the Fryderyk Chopin University Clinical Hospital in Rzeszów. 2.Detailed description of the subject of the contract, the Ordering Party is specified in the Price and Tender Forms - Attachments No. 1 for individual parts, which are an integral part of the SWZ. 3. The Ordering Party requires to offer items of the order that: - meet the requirements described in the Price and Offer Forms, - meet the requirements described in the CVT, - are covered by the Act of 12 May 2011 on the refund of medicines, foodstuffs intended for particular nutritional uses and medical devices – concerns medicinal products covered by the refund. If it is found that a chemical substance in the dose and form indicated in the CVT is included in the Notice of the Minister of Health on the list of reimbursed medicines, foodstuffs intended for particular nutritional uses and medical devices - Annex B and/or C, which is valid as at the date of submission of tenders, and the medicinal product offered is not indicated therein, the tender will be rejected. The offer will also be rejected if the price of the offered medicinal product is higher than the official price indicated in the Notice of the Minister of Health referred to above. 4. The scope of the contract includes the delivery of the items of the contract in the quantities specified in the Price and Offer Forms for the period of: Parts 1 to 3 – 12 months from the date of conclusion of the contract. 5. The Ordering Party in Sections 1 to 3 allows for the possibility of changing the term of the contract concluded with the Contractor in the event of failure to perform the entire subject of the contract during its term. In this case, the amendment of the contract requires the signing of an annex, except that the Contracting Authority shall unilaterally notify the Contractor of the proposed extension of the contract before the expiry of the period for which it was concluded. 6.
