Description

1. Przedmiotem zamówienia jest dostawa endoprotez i implantów ortopedycznych. 2. Przedmiot zamówienia został szczegółowo opisany w Załączniku nr 1 do SWZ. 3. Zamawiający w Załączniku nr 1 do SWZ wymaga podania przez Wykonawcę producenta, nazwy handlowej oraz numeru kodu katalogowego oferowanego przez Wykonawcę przedmiotu zamówienia. 4. Wykonawca musi posiadać Katalogi (zawierające kody katalogowe – REF) oferowanych produktów. 5. Zaoferowany przedmiot zamówienia musi być dopuszczony do obrotu i używania na obszarze Rzeczypospolitej Polskiej zgodnie z wymogami Ustawy z dnia 20 maja 2010 roku o wyrobach medycznych (j.t. Dz. U. z 2020 r., poz. 186) oraz dyrektywami Unii Europejskiej (zgodnie z art. 26 Ustawy z dnia 20 maja 2010 roku o wyrobach medycznych (j.t. Dz. U. z 2020 r., poz. 186). 6. Elementy implantów stanowiące funkcjonalny komplet (np. płytka ze śrubami) muszą pochodzić od tego samego producenta.
