Description

1. The contract concerns:
(a) reconstruction of the premises of the Aesthetics and Intensive Therapy Branch located at the 2nd Central Building, Operational Locator located at the 1st Main Building for the project entitled ‘Increasing the accessibility and quality of health services at the Provincial Specialist Hospital M. Kopernik in Łódź by reconstructing the modernisation of the operating block, the Aesthetic and Intensive Therapy Branch and the Laboratory Diagnostic Institute of the Bank of Krwi and Serology Workshop’,
(b) reconstruction of the Aesthetics and Intensive Therapy Unit, located at the 2nd Central Building,
(C) reconstruction of the Aesthetics and Intensive Therapy Unit located on the 2nd Main Building – Southern part (optional right),
(D) purchase of medical equipment and equipment.
The conditions governing the exercise of the option, which include ordering the execution of works to the extent described in point 1.c, are set out in the model contract.
