Poland-Inowrocław: Disposable non-chemical medical consumables and haematological consumables
1. The subject-matter of the contract is the supply of single-use medical supplies to Dr. L. Błażk Hospital in Inowrocław.
2. A detailed description of the subject-matter of the contract is set out in Annex 2 to the ToR.
3. The contractor must perform the contract on the terms and conditions described in the model contracts constituting Annexes 4, 4a and 4b to the ToR.

6. The contracting authority allows for the possibility of submitting partial tenders
a. The subject matter of the contract is divided and described in 84 lots;
b. Each lot will be referred to in the ToR as ‘Task’, which is designated as task Nos 1 to 65
C. Task No 49 consists of 20 items. The contracting authority allows for the possibility of submitting tenders for individual items in the above task.
D. Each of the tasks is a separate subject-matter of the contract.
e. The scope and size of each task is detailed in: product-price forms, identified and described in the ToR as Annexes 1 and 2 to the ToR, detailing the required range.
F. The contract may be performed as part of the selected task or for the whole
Procurement.
7. The products offered must be accompanied by the required approval certificates for use in the public health service.
8. The quantities shown in Annex 2 represent an estimated annual consumption and may be reduced to 20 % of the unused quantity, without constituting an obligation on the part of the Contracting Authority to implement it in full, nor does the Contractor claim damages for non-utilisation of the maximum value of the contract.
9. The contracting authority does not allow variants to be submitted.
10. The contracting authority shall not provide for the possibility of awarding the contracts referred to in Article 67. Point 1 point 7.
11. The contracting authority does not reserve the obligation for the contractor to carry out work related to the subject-matter of the contract in person.
