Description

1. Przedmiotem zamówienia jest świadczenie usług konsultanta merytorycznego, wykładowcy do prowadzenia szkoleń oraz fizjoterapeutów do prowadzenia warsztatów rehabilitacji ruchowej w ramach projektu pn. „Priorytet kręgosłup” (CPV: 80561000-4 Usługi szkolenia w dziedzinie zdrowia, 80511000-9 Usługi szkolenia personelu, 85142100-7 Usługi fizjoterapii). Zamówienie składa się z 5 zadań.
2. Szczegółowy opis przedmiotu zamówienia określający wymagania Zamawiającego, zakres oraz sposób jego realizacji zawarte są w następujących załącznikach:
1) Zał. nr 1A do SIWZ – formularz cenowy;
2) Zał. nr 1B do SIWZ – opis przedmiotu zamówienia (OPZ);
3) Zał. nr 1C do SIWZ – projekty umów.
3. W przypadku opisu przedmiotu zamówienia (zał. nr 1B i 1C do SIWZ) przez odniesienie do norm, europejskich ocen technicznych, aprobat, specyfikacji technicznych i systemów referencji technicznych, o których mowa w art. 30 ust. 1 pkt 2 i ust. 3 ustawy Pzp, Zamawiający dopuszcza rozwiązania równoważne opisywanym.
