With regard to the condition set out in Article 100 of the Public Procurement Act concerning accessibility for disabled persons referred to in Directive 2014/24/EU of the European Parliament and of the Council of 26 February 2014 on public procurement (OJ L 94, p. 65), the transport of these persons will be carried out by other vehicles adapted to such transport as part of equipment owned by the Police or through medical assistance authorised to transport disabled persons. There are no activities related to the performance of the contract consisting in performing work in the manner specified in Article 22(1) of the Act of 26 June 1974 – Labour Code – one-off delivery. The delivery date of the order is: up to 5 calendar days from the date of signing the contract. Contractors applying for the award of a contract are obliged to attach to the tender submitted the means of proof in question, confirming that the offered delivery meets the requirements, characteristics and criteria specified by the Contracting Authority. 1. List of the means of proof in question to be attached to the tender submitted: 1.1. Extract from the vehicle type-approval certificate for the base vehicle or EC certificate of conformity for complete vehicles, confirming the vehicle parameters used as a basis for calculating the points in the non-price criteria. The contracting authority requiring the following to be attached to the tender: "Document attesting compliance with the requirement (EC certificate of conformity of the base vehicle) (.......) shall mean the representation of a model of such document for a vehicle which is the same (same make and type and having the same technical characteristics to be assessed under the non-price criteria) as the one offered. Due to the fact that the criteria for the evaluation of the offer are, among others, the parameters of the vehicle disclosed in this document, the Contracting Authority requires that it be attached to the offer for comparison and assessment of the parameters of the offered vehicle. Due to the fact that the indicated document confirms the parameters of the offered vehicle (description of the subject of the contract), which are criteria for the evaluation of tenders, this document cannot be supplemented (it is the content of the tender). 1.2. A statement confirming that the offered supplies meet the requirements set out in Annexes 1, 1.1, 1.2 to the SWZ, made by filling in and signing Annexes 1.1, 1.2 to the SWZ. If the Contractor, when submitting a tender in more than one part of the procedure, offers the same make and model of vehicle, he shall attach to the tender only one extract from the vehicle type-approval certificate for the base vehicle or the EC certificate of conformity for complete vehicles, which will confirm that all offered vehicles meet the minimum requirements described by the Contracting Authority. The same principle applies to statements constituting the means of proof in question (declarations in Annexes 1.1 and 1.2 to the SWZ). if the Contractor submits a tender for more than one part of the procedure and offers for those parts the supply of vehicles of the same make in the same model, the declarations shall be submitted in one copy only. If, on the other hand, the Contractor submits a tender in more than one part of the procedure and offers a different make/model of the vehicle in each part, the means of proof in question (extract from the vehicle type-approval certificate for the base vehicle or the EC certificate of conformity for complete vehicles and statements from Annexes 1.1 and 1.2 to the RVU) shall be submitted separately for each part of the procedure in which he submits a tender (they confirm compliance of the offered supply with the requirements of the Ordering Party described in the RVU). Form of the evidence in question: Chapter VI SWZ. Supplementing and clarifying the content of the evidence in question: - The extract from the vehicle type-approval certificate for the base vehicle or the EC certificate of conformity for complete vehicles shall not be supplemented. - If the IPC does not submit the statements constituting the means of evidence referred to in Chapter VI, point 1.2, of the SWZ (above) or the means of evidence submitted are incomplete, the Contracting Authority will call for them to be submitted or supplemented within the prescribed time limit. - Zam. may request explanations from contractors concerning the content of the means of proof in question. 7.
