Description

1. The subject of the contract is the task entitled: ‘Supply of disposable medical equipment for clinics/branches of CSK UM in Łódź’ 2. CPV Common Procurement Vocabulary: 33140000-3 Medical materials 3. The detailed description and the Assortment-Price Form are set out in Annex 2 to the Specifications of the Contract Terms and Conditions (SWZ). 4. The Ordering Party has divided the order into individual parts and allows the possibility of submitting partial offers for any number of Packages - Packages: No 1 – No 2 5. The subject-matter of the contract must meet the requirements specified by the Ordering Party in the Terms and Conditions Specifications, in particular the conditions set out in the description of the subject-matter of the contract in Annex 2 – Assortment and price form. Failure to meet at least one of the limit conditions specified in the above-mentioned Annexes will result in the rejection of the tender from the procedure.
