1) Przedmiot zamówienia stanowi świadczenie usług transportu sanitarnego w zakresie – przewozów sanitarnych karetkami typu „T”, „K”, „P” i „S” 2) Przedmiot zamówienia został podzielony na 2 zadania (części). - USŁUGI TRANSPORTU SANITARNEGO KARETKAMI SPECJALISTYCZNYMI TYPU S - USŁUGI TRANSPORTU SANITARNEGO W OPCJI T, K, P 3) Wykonawcy mogą składać oferty na dowolnie wybrane zadanie. Każdy wykonawca może złożyć ofertę obejmującą jedną część zamówienia, lub dowolnie wybraną większą liczbę części zamówienia. 4) Szczegółowy opis przedmiotu zamówienia, w tym wszystkie zadania (części) oraz opis wymagań stawianych przez zamawiającego w zakresie realizacji poszczególnych zakresów świadczeń, określają: - Opis przedmiotu zamówienia – Załącznik nr 1 do SWZ - Formularze cenowe dla Zadania nr 1 i 2, tj. odpowiednio – załączniki nr 2a i 2b do SWZ, - projekt umowy na świadczenie usługi transportu sanitarnego dla Zadania nr 1 Karetkami specjalistycznymi typu S – załącznik nr 6a do SWZ. - projekt umowy na świadczenie usługi transportu sanitarnego dla Zadania nr 2 w opcji T, K, P – załącznik nr 6b do SWZ.
