Description

1. The subject-matter of the contract is the purchase and successive delivery of oncological medicines and miscellaneous medicines, listed in sections for the Fryderyk Chopin University Clinical Hospital in Rzeszów. 2. Detailed description of the subject of the contract, the requirements of the Ordering Party are specified in the Price and Tender Forms – Attachments No. 1 for individual parts, which are an integral part of the SWZ. 3. The scope of the contract includes the delivery of the items of the contract in the quantities specified in the Price and Offer Forms for the period of: Parts 1 to 29 – 12 months. 4. The contracting authority shall not allow equivalent tenders to be submitted. 5. DEADLINE FOR COMPLETION OF THE CONTRACT: Purchase and successive delivery of order items: Part from No. 1 to No. 14 - for 12 months, counting from 11.11.2024. Part from No. 15 to No. 25 - for 12 months, counting from 22.11.2024. Part from No. 26 to No. 29 - for 12 months, counting from 06.12.2024. In Part No. 1 to No. 29, the Ordering Party allows the possibility of making purchases earlier in the event of exhaustion of the quantity from the current contract. In this case, the term of the contract will be counted from the moment of the first purchase of the subject of the contract. The contracting authority in Parts 1 to 29 allows for the possibility of later commencement of the contract in the event of prolongation of the tender procedure. The Ordering Party in Sections 1 to 29 allows for the possibility of changing the term of the contract concluded with the Contractor in the event of failure to perform the entire subject of the contract during its term. In this case, the amendment of the contract requires the signing of an annex, except that the Contracting Authority shall unilaterally notify the Contractor of the proposed extension of the contract before the expiry of the period for which it was concluded.
