Description

1. Przedmiotem zamówienia są sukcesywne dostawy produktów farmaceutycznych do Apteki Zakładowej Aresztu Śledczego w Gdańsku, z podziałem na 8 części, w asortymencie i w ilościach szacunkowych wskazanych w załączniku nr 1 do specyfikacji, który stanowi jednocześnie formularz do sporządzenia oferty. 2. Przedmiot zamówienia musi być dopuszczony do obrotu na terytorium Rzeczpospolitej Polskiej zgodnie z Obwieszczeniem Prezesa Urzędu Rejestracji Produktów Leczniczych, Wyrobów Medycznych i Produktów Biobójczych z dnia 18 grudnia 2023 r. w sprawie ogłoszenia Urzędowego Wykazu Produktów Leczniczych Dopuszczonych do Obrotu na Terytorium Rzeczypospolitej Polskiej (Dz. Urz. MZ z 2023 poz. 115) oraz spełniać warunki Ustawy o wyrobach medycznych z dnia 7 kwietnia 2022 r. (Dz. Urz. MZ z 2022 poz. 974 z późn. zm.).
