Description

1. The subject-matter of the contract is the delivery to Apteki of the Voivodship Hospital in Bielsko-Biała at Al. The National Army 101 pharmaceutical products and single-use items for newborn babies in accordance with the quantitative and generic list set out in Annex 2a to the Specifications of Essential Contract Conditions, hereinafter referred to as ‘SIWZ’, under the conditions laid down in the model contract constituting Annex 4 to the ToR.
2. The procurer shall allow equivalent pharmaceutical products to be offered. An equivalent product shall be considered to be a product containing the same active substance, at the same dose, with the same indications for use and the same pharmaceutical form.
