Description

1. The subject of the contract is the supply of medical devices used in neurosurgery for UCK in the assortment, estimated quantity and requirements specified in Annex 3 to the FTZ. 2. The contracting authority shall allow partial tenders to be submitted. Partial tenders are defined as individual packages set out in Annex 3 to the FTZ. The Contractor may submit an offer for one, two, three or four packages. 3. The Ordering Party requires that the offered assortment meets the requirements for medical devices contained in the Act of 7 April 2022 on medical devices (Journal of Laws of 2022, No. Journal of Laws 2024, item 1620, i.e.) and in implementing acts issued on the basis of this Act. 4. The range offered must meet the requirements set out in Annex 3 to the FTZ. Failure to meet even one of the requirements set out in Annex 3 to the FTZ will result in the rejection of the tender. 5. The delivery of the assortment of this order will be carried out on the basis of partial orders. The maximum delivery time is 6 working days from the date of placing the order by the Ordering Party. 6. Assortment referred to in paragraph. 1 The Contractor shall be obliged to deliver at his own expense to the Medical Supplies Department, ul. Smoluchowski 17.
