Description

1. The subject-matter of the contract is the supply of medicines, substances for formulation and various pharmacy products – Supplement II, broken down into 35 tasks.
2. The form, doses and quantities of the subject-matter of the contract are specified in detail in the product price form, which is attached as Annex 1 to the ToR.
3. Time limit for delivery – according to current needs, in each case within a maximum of 2 working days from the date on which the order was placed.
4. Series and expiry date on the unit packet and on the invoice.
5. A period of validity of the pharmaceutical products offered not less than 12 months from the date of delivery to the Contracting Authority.
6. The contractor will deliver the products to the designated Magazine Apteki Hospital.
7. The pharmaceutical products offered must be authorised in Poland for each form and dose required separately.
