Description

1. The subject of the contract is ‘Supply of medical equipment II’ under the project entitled ‘Supply of medical equipment II’. "Creation of the Świętokrzyskie Geriatrics Center as a supraregional interdisciplinary clinical center unique for south-eastern Poland for patients with multimorbidity of the elderly"; co-financed under the National Recovery and Resilience Plan - Component D Investment D4.1.1.’. 2. The scope of the contract includes delivery, assembly with the required installations (possible adaptation of the room, if necessary) along with commissioning and training of the Employer's employees. 3. The goods offered by the Contractor must: a) Have valid documents allowing the release for free circulation on the territory of the Republic of Poland in accordance with the provisions of the applicable Act of 7 April 2022 on medical devices and the method of classification on the basis of the Regulation of the Minister of Health of 5 November 2010 on the method of classifying medical devices - if it concerns a given device. b) meet the requirements specified by the Ordering Party in the Terms and Conditions of the Order, in particular the conditions specified in the description of the subject of the contract - annexes with the required technical parameters. c) be brand new, fully functional, not remanufactured, not demo version. d) be produced at least in 2025, e) meet the quality and technical standards resulting from the function and purpose, f) be free from defects in materials, construction, physical and legal, g) may not be burdened with any rights for third parties. 4. The Offered Item of the Order must be complete with all components, parts and materials necessary for commissioning and use. The offered subject of the contract must meet the requirements of the Ordering Party. 5. The description of the offered product should not raise any doubts of the Ordering Party. The description should indicate that the offered subject of the contract is of the same parameters as required by the Ordering Party or better, by accurately indicating the parameters of the equipment offered. 6. The detailed description of the subject of the contract includes this FTZ and: – Annex 2 – Technical parameters required – Packages 1 and 2 – Annex 3 – Assortment price form – Packages 1 and 2 – Annex 4 – Draft contract.
