Description

1.1. The subject of the contract is the supply of medical devices for orthopaedics, in accordance with the following lots: task 1 Task 2 Task 3 Task 4 Task 5 Task 6 Task 7 Task 8 Task 9 Task 11 Task 12 Task 13 Task 14 Task 15 Task 16 Task 17. Place of delivery of the above supplies – Contracting Authority at ul. Gen. A. E. Fieldorf 2 in Wrocław. Those parts of the subject-matter of the contract are indicated and described in the ‘Price Formula’, which forms an integral part of the Specifications for the Relevant Conditions of the Contract. 1.2. For Task 1, Task 2, Task 3, Task 4, Task 5, Task 6, Task 7, Task 8, Task 9, Task 10, Task 12, Task 14, Task 16, the Contractor shall make available to the Contracting Authority the set of instruments necessary to implant the devices specified in the individual parts of the subject-matter covered by the contract. This will be made available by leasing the above facility by the Contracting Contractor for the duration of the contract or by renting the above-mentioned instrumentar for the duration of the operation. In the case of Task 15, the Contractor will make available a centrifuge to the Contracting Authority. The rules for making available are described in the model contracts, which form an integral part of the ToR. The content of these lists will be determined in detail after the selection of the most advantageous tender and before the conclusion of the public contract. 1.3. The procurer will set up a ‘orthopaedic medical device bank’ from a selection of medical devices, comprising between 2 and 4 sets. The contents of the bank will be determined in detail after the selection of the most advantageous tender and before the conclusion of the public contract. The goods forming part of the bank will be the property of the Contractor. The use of the above-mentioned products for the operation will mean their purchase by the Contracting Authority. The parts of the subject-matter of the contract for which a bank will be set up are indicated in the ‘Price Formula’, which forms an integral part of the ToR. 1.4. In the case of Task 2, Task 5, Task 6 and Task 7, the Contractor whose tender was selected by the Contracting Authority as the most advantageous before the conclusion of the trans-tender contract will evaluate the individual elements of the selected sets of medical devices, as indicated in the ‘Price Form’, which forms an integral part of the ToR. This detailed valuation of the individual elements of the sets will be shown in Annex 1 to the contract. The sum of the prices of each of the components of the set must not exceed the price of the whole set, as indicated by the Contractor in the ‘Price Formula’ submitted in the tender. 1.5. The contractual conditions for the performance of the public contract in question contain model contracts, which form an integral part of the ToR.
