Description

The subject-matter of the contract covers the provision of health services to the National Bank of Poland in the following areas:
1. Occupational medicine
2. The provision of health services to workers and other qualified persons who have subscribed to one of the following packages, i.e.:
(1) staff,
(2) family,
(3) partner,
(4) pensioner.
3. The provision of health services packaged in points (1) to (3) of point 2 shall include:
1) basic medical consultation
(2) consultation of specialist medical care,
(3) outpatient treatments,
(4) laboratory tests,
(5) diagnostic tests,
(6) home visits,
(7) medical and rehabilitation treatments,
(8) pregnancy,
(9) prophylactic dental care,
(10) one day surgery,
(11) protective influenza vaccination,
(12) prevention consisting of carrying out an annual review of the entitled person’s state of health:
demand – depending on age, sex, work performed.
4. The provision of health services as part of the package set out in point 2(4) shall include:
1) basic medical consultation
(2) consultation of specialist medical care,
(3) outpatient treatments,
(4) laboratory tests,
(5) diagnostic tests,
(6) home visits,
5. Running at the National Head Office of the Polish Bank in Warsaw, at ul. Świętokrzyskie 11/21 of the following specialist offices, which are used by doctors: occupational internist/medicine, ophthalmologist, gynaecologist, cardiologist, orthopaedician, dentist, and cabinets: interventional and physicotherapy.
6. Provision of medical first aid in the event of a situation of sudden deterioration in the state of health on the premises of the National Bank of Poland, e.g.: sudden syndromes, loss of consciousness, serious bodily injury, severe bleeding.
7. The provision of health services will be carried out in the territory of the Republic of Poland, including, on a mandatory basis, within the administrative boundaries of cities: Białystok, Bydgoszcz, Gdańsk, Katowice, Kielce, Kraków, Lublin, Łódź, Olsztyn, Opole, Poznań, Rzeszów, Szczecin, Warsaw, Wrocław, Zielona Góra.
