Description

The subject-matter of this tendering procedure is: Purchase of a Physician Assistant integrated into the Clininet HIS system (‘the Software’) in accordance with the type of assortment and the quantities specified in the ‘Assortment and Price Form’ attached as Annex 2 to the SPC and in accordance with the ‘Offer Form’ attached as Annex 1 to the SPC and the Description of the subject-matter of the Order attached as Annex 1.a and Annex 1 b – Conditions of Guarantee and Service for the Independent Public Health Care Unit of the University Clinical Hospital No 2 of the Medical University of Łódź, ul. Żeromskiego 113, 90-549 Łódź, Independent Public Health Care Unit of the Norbert Barlicki University Clinical Hospital No 1, Medical University of Łódź, ul. Kopcińskiego 22, 90-153 Łódź and the Independent Public Health Care Department of the Central Clinical Hospital of the Medical University of Łódź, ul. Pomorska 251, 92-213 Łódź. The Assortment and Price Form, in the appropriate columns, indicates the quantities to be ordered by the Independent Public Health Care Unit of the University Clinical Hospital No 2 of the Medical University of Lodz, the Independent Public Health Care Unit of the University Clinical Hospital No 1 of the Norbert Barlicki Medical University of Lodz and the Independent Public Health Care Unit of the Central Clinical Hospital of the Medical University of Lodz. Each of the Ordering Parties signs a separate contract for the award of the contract in terms of the quantities specified in the Assortment and Price Form for a given hospital. The implementation of the subject of the proceedings is co-financed by the Medical Research Agency as part of competition No. ABM/2023/2 Creation and development of Regional Centres for Digital Medicine. Project name: Medical University of Lodz – Digital Medicine Center (MULDiMediC ).
