Description

Przedmiotem zamówienia jest dostawa produktów leczniczych, leków do chemioterapii, kontrastów. Zamawiający dopuszcza możliwość składania ofert częściowych, w podziale na 15 części. Szczegółowe wymagania dotyczące realizacji zostały określone w tabeli asortymentowo-cenowej we Wzorze Umowy, stanowiącymi odpowiednio Zał. nr 2 i 3 do SWZ. Szczegółowy opis oraz sposób realizacji zamówienia zawiera wzór umowy, stanowiący Zał. nr 3 do SWZ. Zamawiający oświadcza, iż będąc administratorem danych oraz mając na względzie przepisy ustawy z 10.5.2018 o ochronie danych osobowych (dalej RODO) wdrożył środki bezpieczeństwa adekwatne do zidentyfikowanych przez siebie ryzyk oraz dające rękojmie należytego zabezpieczenia danych osobowych przekazywanych w ramach postępowania. Szczegółowa klauzula informacyjna z art. 13 RODO zawarta jest w SWZ.
