1) The subject of the contract is the provision of training services in the field of change management (4 hours) and process management (4 hours), a total of 8 hours for an average group of 20 people (min. 16 people, max. 24 people) for employees of public psychological and pedagogical clinics, carried out stationary on one day with the provision of a room and meals for participants, i.e. coffee break and lunch (in the case of the Silesian Voivodeship, excluding coffee break). 2) The scope of the basic contract includes the provision of training services in the number of: Part A: Kuyavian-Pomeranian Voivodeship, 56 training hours, 7 training groups, max. 168 people. b) Part B: Lubelskie Voivodship, 80 training hours, 10 training groups, max. 240 people. c) Part C: Lubuskie Province, 40 training hours, 5 training groups, max. 120 people. (d) Part D: Lodz Voivodeship, 80 training hours, 10 training groups, max. 240 people. (e) Part E: Małopolskie Province, 80 training hours, 10 training groups, max. 240 people. f) Part F: Opole Province, 32 training hours, 4 training groups, max. 96 people. (g) Part G: Pomeranian Voivodeship, 56 training hours, 7 training groups, max. 168 people. h) Part H: Silesian Voivodeship, 112 training hours, 14 training groups, max. 336 people. The detailed scope of services covered by the option right is set out in Annex 2 to the FTZ. (b) Details of the option, specifying the type and circumstances of exercising the option right are set out in Annex 3 to the FTZ (model contract). 10) Deadline for the execution of the order: up to 5 months from the date of conclusion of the Agreement no later than 31.03.2026, provided that the performance of the Subject of the Agreement within the scope of the part covered by the option may not exceed the date of completion of the Agreement.
