1. The subject-matter of the contract is the supply of peripheral cannulae and ophthalmic probes for USK-2 PUM in Szczecin (‘the products’). 2. The estimated quantities of products and the description of products are set out in the unit price form (Annex I). 2 to the SWZ, hereinafter also referred to as FCJ). 3. The contracting authority requires the offer of medical devices within the meaning of the Medical Devices Act (Journal of Laws 2024.1620, as amended), hereinafter: “UoWM”, placed on the market and used in the territory of the Republic of Poland in accordance with the applicable regulations. 4. The Ordering Party requires that the delivered products have a shelf life of not less than 24 months, counting from the date of their delivery to the Ordering Party's registered office. 5. The Ordering Party has the right to order a smaller quantity of products, with the exception that not less than 60% of the quantity specified in the unit price form. 6. The order consists of 3 tasks. 7. The contracting authority shall not allow variants to be submitted. 8.The Ordering Party requires the implementation of the subject of the order: a) For Task No. 1 for a period of 36 months; b) For Task No. 2 for a period of 24 months; c) For task No. 3 for a period of 12 months from the date of conclusion of the contract. 9.The Ordering Party requires the submission of the means of evidence in question in the form of samples for Task No. 1 - in accordance with the above.
