Description

The Program for the prevention and early detection of cervical cancer has a long history of operation in our autonomous community. The Programme is included in the portfolio of services of the National Health System by Order SCB/480/2019 of 26 April 2019 amending Annexes I, III and VI to Royal Decree 1030/2006 of 15 September 2006 and establishing the portfolio of common services of the National Health System and the procedure for its updating, which establishes the obligation of its offer to the target population and the general lines that it must comply with. The offer to the target population of this Program is based on the population benefit of the early treatment of cervical injury detected early on in the reduction of mortality due to cervical cancer. Likewise, the Pro-grama as an organized structure, guarantees the coordination, quality and equity of the screening process, has a wide acceptance in the population and in health professionals. It thus contributes to the rationalisation and optimisation of the Community's health resources. The General Directorate of Public Health of the Ministry of Health has carried out in the last three years a series of updates of the Program in order to adapt it to the criteria established in Order SCB/480/2019, of April 26, already mentioned above and in which the population screening of cervical cancer is oriented with the following configuration: Target population: women aged between 25 and 65 years. (b) Primary screening test and interval between scans: 1st Women aged between 25 and 29 years: i. Women without adequate protection from HPV vaccination: cytology every 3 years. ii. Women with adequate protection from HPV vaccination: depending on the state of implementation of the screening programme, cytology shall be carried out every 3 years or screening shall start at the age of 30. 2nd Women aged between 30 and 65 years: determination of high-risk human papillomavirus (HPV-AR), regardless of HPV vaccine status: i. If HPV-AR is negative: repeat HPV-AR test at age five. ii. If HPV-AR is positive: appropriate triage for stratification of the risk of HPV injury. If high risk is ruled out, repeat HPV-AR annually. Over the next two years: the programme does not need to be amended either for the two age ranges of the target population (25-29 years and 30-64 years), or for primary screening tests (liquid cytology: 25 to 29 years, and HPV determination: from 30 to 64 years). The program is based on the determination of HPV-AR and in our Autonomous Community, it is the primary choice test in women between 30 and 65 years old (formerly 35-65) and for women between 25 and 29 years old (formerly 25-34) in those cases in which cytology has a non-determining result. In turn, the Program in Castilla y León, is based on the individual risk that each woman presents of suffering from cervical cancer. Thus, there are women with intermediate risk data whose indication is the repetition of both screening tests per year. The indication of the performance of both screening tests (Citology + HPV) is called co-test and it is recommended to perform it in the same laboratory consecutively regardless of the result of each of them to ensure the efficiency of the screening tests.
