Description

The subject of the contract is the supply of medical devices specified in detail in Annex 1 to the FTZ. The equipment must be unused and brand new. Year of production - not earlier than 2025. The offered subject of the contract must comply with the provisions of law in force in the European Union as well as in Poland, must have certificates, attestations, and be allowed to be used in Poland. In addition, in accordance with Article 18 of the Act of 7 April 2022 on medical devices, the Ordering Party requires the presence of a UDI on labels and higher levels of packaging, if these codes have been assigned in accordance with Article 27 sec. 8 of Regulation (EU) 2017/745 and Article 24(1)(b) and (c) of Regulation (EU) 2017/745. 8 of Regulation 2017/746. All documents concerning UDIs should be in the possession of the CONTRACTOR and ready to be submitted at any request of the Employer. The detailed description and requirements concerning the subject of the contract are attached as Annex 1 to the FTZ. 2. The subject of the contract has been divided into lots. The contracting authority shall allow tenders to be submitted in the following parts: • Part 1 – supply of clinical defibrillators • Part 2 – supply of automatic chest compression devices • Part 3 – supply of rescue and transport ventilators
