1. The subject-matter of the contract is the supply of medical equipment for highly specialised imaging diagnostics to the Provincial Hospital Zespolated in Toruń, together with the adaptation of the premises:
— task 1: a computed tomography apparatus, at least 64 in row, at least 128 layered,
—task 2: digital x-ray apparatus with fluoroscopy – 1 unit,
task No 3: digital mammography apparatus with tomosynthesis – 1.
1.1. The required technical and utility parameters of the medical equipment ordered, the scope of any additional services, the guarantee and service conditions required are set out in Annexes 3/1 to 3/3 to the ToR, which the Contractor must complete and attach to the tender.
1.2. At its own expense, the Contractor will be required to deliver the equipment to the Contracting Authority’s premises, install and assemble it, train the Contracting Authority’s staff in the proper handling, submit on the day of delivery a guarantee card and a manual for use in Polish.
1.3. The contractor will carry out the subject matter of the contract within the scope and in the manner specified in the ToR, and the adaptation work of the works works in accordance with the mandatory rules, standards and construction works on the basis of a visual inspection of the site of the works and with the utmost care. Before submitting a tender (in order to properly assess the scope of the contract and the costs of risk) the contractor may consult all the conditions for the location and implementation of the adaptation works in order to take them into account.

1.4. The fitting-out of the premises shall also include appropriate finishes for the intended purpose of the premises, including:
— floor floors as recommended by the equipment manufacturer, preheated PVC lining with an anti-electrostatic certificate for use in healthcare facilities with increased abrasion strength with a wheeled wall, walls complying with the requirements of the individual workstations, colors of finishing materials, contractor shall agree with the procurer during the task, paints authorised for use in healthcare facilities with appropriate attestations, door width in accordance with the applicable regulations, proper marking of the doors in the workshop, construction of rubbishes shall be used for painting. The contractor will provide adequate protection against X-rays of the apparatus and, after execution, provide the contracting authority with appropriate documents proving the effectiveness of the security features.
1.5. The contractor will carry out the adaptation work under the supervision of suitably qualified persons.
1.6. The contractor will carry out the assembly, installation of equipment and other construction works necessary for the start of the workstation in accordance with the Regulation of the Minister for Health of 21.8.2006 on detailed conditions for safe work with radiological equipment (Journal of Laws No 180, item 1325).
All necessary documentation and adaptation work must be carried out in accordance with the legislation in force:
— The Construction Act of 7.7.1994,
— Regulation of the Minister for Health of 10 November 2006 on the professional and sanitary requirements to be met by healthcare facilities and facilities (Journal of Laws 2006, No 213, item. 1568 as amended),
— The Atomic Law Act of 29 November 2000 (Journal of Laws 2007, No 42, item 276),
— Cabinet Regulation of 12.7.2006 laying down detailed conditions for safe work with ionising radiation sources (Journal of Laws 2006/140, item 994),
— Regulation of the Minister for Health of 27.3.2008 laying down minimum requirements for health care units providing health services in the field of X-ray diagnosis, interventional radiology and diagnosis and therapy of radioisotopic non-cancer diseases (Journal of Laws 2008, No 59, item 365),
— Health and safety requirements appropriate to the task to be carried out.
1.7. The medical apparatus which is the subject of the contract must comply with the legal requirements and meet the technical standards laid down for them.
1.8. The adaptation works will be carried out from the contractor’s materials.
1.9. The contractor will provide the works protocols after the execution of the contract, together with all the required and necessary documents relating to the operation of the works plant, without additional investment from the contracting authority, including the approval of the Provincial Sanitary Inspector.
1.10. The Contractor must carry out himself documentation of the fixed enclosures in which medical equipment, ventilation and air-conditioning design will be installed and the tests necessary for acceptance of the works facility by the Voivodship Sanitary and Epidemiological Station.
1.11. The contractor will be required to complete the as-built documentation.
1.12. The contractor carrying out Task 1 must implement and install information and souvenir plaques in accordance with the guidelines:

2. The duration of the warranty period for the equipment offered – the Contractor shall specify in the tender form and annexes 3/1 to 3/3 (but not less than 24 months from the date of acceptance of the subject of the contract).
3. The required warranty period for the adaptation works carried out – 24 months from the date of acceptance of the subject of the contract.
4. The contracting authority does not provide for the possibility of awarding complementary contracts as referred to in Article 67 of the Public Procurement Act.
