Description

The subject of the contract is: Purchase of medical equipment and apparatus for the needs of the Ophthalmological Department of the SPZOZ Provincial Specialist Hospital No. 3 in Rybnik. The delivery specifications are set out in Annex 1.1, i.e. ‘Price form’. The other terms of the contract are set out in Annex 2 to the tender specifications (‘Draft provisions of the contract’, on public procurement, which will be incorporated into the content of the contract). The order is divided into 6 packages (parts). Agreements on the settled packages will be concluded at the end of the present proceedings. A detailed description of the subject of the contract is included in the annexes: 1) Price forms (separately for each package) – Annex No 1.1a – 1.1b to the FTZ, 2) Technical specifications (separately for each package) – Annex No 1.2 a – 1.2 b to the FTZ, 3) Quality and service guarantee conditions – Annex No 1.3 to the FTZ, 4) Draft contract provisions – Annex No 2 to the FTZ. The task received funding from the budget of the Silesian Voivodeship. Pursuant to Article 257 of the Public Procurement Act, the contracting authority reserves the right to cancel the procurement procedure if the public funds it intends to allocate to finance all or part of the contract are not granted to it. The Contracting Authority informs that, in accordance with Article 139 of the Public Procurement Law, it will first examine and evaluate the tenders and then make a qualitative selection of the Contractor whose tender was the highest rated, as regards the lack of grounds for exclusion and compliance with the conditions for participation in the procedure. In compliance with the obligation under Art. 6 of the Act of 14 June 2024 on the protection of whistleblowers (Journal of Laws 2024, item 928) the contracting authority informs that the Independent Public Health Care Department of the Provincial Specialist Hospital No. 3 in Rybnik has adopted a procedure for internal reporting information on violations of the law and taking follow-up actions.
