Description

1.The subject of the contract are health services, which will be carried out within the Ophthalmology Department of the Municipal Hospital in Ruda Śląska Sp. z o.o. according to the needs of the Awarding Party in accordance with the Department's Work Schedule. The term ‘health services’ should be understood to mean all activities aimed at preserving, rescuing, restoring and improving health and other medical activities resulting from the treatment process or separate provisions regulating the rules for their performance. 2.The subject of the order has been divided into 5 packages: Package I Healthcare services will be provided within the Ophthalmology Branch in conjunction with the performance of the duties of the Branch Chief according to the needs of the Awarding Branch in accordance with the Branch's Work Schedule approved by the Director for Health and Safety. Medical: a) from Monday to Friday between 07:25 and 15:00 (minimum 55 hours per month); b) from Monday to Friday between 15:00 and 07:25 of the following day (average monthly 5 benefits); c) on Saturdays, Sundays and public holidays (holidays) between 07:25 and 07:25 of the following day (average monthly 2 benefits); d) performed cataract surgery (average monthly 120 treatments); e) performing antiglaucoma surgeries / laser treatments (average monthly 8 treatments); f) performing phacotraubeculectomy treatments (average monthly 4 treatments). The doctor providing services must have the title of specialist in ophthalmology / eye diseases and a minimum of 2 years of experience in the function of the Head of the Ophthalmology Department. As part of package I, the Awarding Entity allows the selection of a maximum of 1 (one) most advantageous tender that meets all the conditions required by the Awarding Entity. Package II Healthcare services will be provided within the Ophthalmology Department according to the needs of the Awarding Body in accordance with the Department's Work Schedule approved by the Branch Chief/Director for Health and Safety. Medical: a) Monday to Friday between 15:00 and 07:25 the following day (4 benefits on average monthly); b) Saturdays, Sundays and public holidays (holidays) between 07:25 and 07:25 the following day (2 benefits on average monthly). The doctor providing the services must hold the title of specialist in ophthalmology/eye diseases. As part of package II, the Awarding Entity allows the selection of a maximum of 1 (one) most advantageous tender that meets all the conditions required by the Awarding Entity. Package III Healthcare services will be provided within the Ophthalmology Department according to the needs of the Awarding Body in accordance with the Department's Work Schedule approved by the Branch Chief/Director for Health and Safety. Medical: a) from Monday to Friday between 15:00 and 07:25 of the following day (average monthly 3 benefits); b) on Saturdays, Sundays and public holidays (holidays) between 07:25 and 07:25 of the following day (average monthly 1 benefit). The doctor providing the services must be at least in the course of specialisation in ophthalmology/eye diseases. As part of package III, the Awarding Entity allows the selection of up to 2 (two) most advantageous tenders that meet all the conditions required by the Awarding Entity. Package IV Healthcare services will be provided within the Ophthalmology Department according to the needs of the Awarding Body in accordance with the Department's Work Schedule approved by the Branch Chief/Director for Health and Safety. Medical: a) from Monday to Friday between 07:25 and 15:00 (average monthly 20 benefits); b) from Monday to Friday between 15:00 and 07:25 of the following day (average monthly 2 benefits); c) on Saturdays, Sundays and public holidays (holidays) between 07:25 and 07:25 of the following day (average monthly 1 benefit); d) performing antiglaucoma surgeries / laser treatments (average monthly 6 treatments). The doctor providing the services must hold the title of specialist in ophthalmology/eye diseases. As part of package IV, the Awarding Entity allows the selection of a maximum of 1 (one) most advantageous tender that meets all the conditions required by the Awarding Entity.
