Description

1. The subject matter of the contract is the supply of orthopaedic implants and has been divided into six separate packages as defined in Annex 1 to the ToR.
2. A detailed description of the subject-matter of the contract, including information on the composition, quantities and requirements thereof, is set out in the price form – which forms Annex 1 to the ToR – in which the Contracting Authority also requires the name of the manufacturer and the catalogue number of the subject-matter of the contract in order to verify the correctness of the individual supplies.
3. The subject of the contract offered must be allowed to be marketed and used in Poland in accordance with the requirements of the Medical Devices Act of 20 May 2010 (Journal of Laws 2015, item 876) and European Union directives in accordance with Article 26 of the Medical Devices Act of 20.5.2010.
4. The period of validity of the range offered shall not be less than 12 months from the date of each delivery.
5. Transport at the Contractor’s expense and risk.
6. The contractor is required to provide systematic training to the Contracting Authority’s staff in the application of the proposed subject-matter of the contract throughout the duration of the contract.
