1. The subject of the contract is the provision to the Provincial Hospital in Bielsko-Biała, located at Al. Armii Krajowej 101, 43-316 Bielsko-Biała, transport services, including: 1) provision of patient transport services for the Provincial Hospital in Bielsko-Biała – Part 1, 2) provision of patient transport services for biological material and personnel of the Contracting Authority for the Provincial Hospital in Bielsko-Biała – Part 2, 3) provision of patient transport services for haemodialysis and kidney transplantation for the Provincial Hospital in Bielsko-Biała – Part 3. 2. The detailed scope of the subject-matter of the contract shall specify: 1) description of the subject-matter of the contract constituting Annex No 2 to the Specifications, hereinafter referred to as "swz", 2) draft provisions of the contract for each part of the contract constituting Annexes No 6.1-6.3 to swz. 3.
