The subject of the order is the lease of 14 devices for performing continuous renal replacement therapies along with the delivery of disposable consumables and dialysis fluids for the needs of USK-2 PUM 1.1 Clinics. Whenever in the further part of the SWZ I will use the phrase: a) "products" - this term should be understood as consumables indicated in Annex 2 (FCJ) b) "equipment" - this term should be understood as leased equipment in task 1C , the description of which is contained in Annex 2 (FCJ ) 2.Estimated quantities of products and a detailed description of the subject of the contract together with the required parameters form is contained in Annex 2 to the SWZ (FCJ ) 3. For task 1, part A, the Contracting Authority requires the offer of medical devices – within the meaning of the Medical Devices Act of 7 April 2022 (Journal of Laws 2022, item 974 as amended. ), hereinafter: ‘UoWM’), placed on the market and used in the territory of the Republic of Poland in accordance with the applicable provisions. For task 1, part B, the Contracting Authority requires the offer of medicinal products in accordance with the Pharmaceutical Law Act of 6 September 2001 4. The quantities of products given in the unit price form are indicative and the Ordering Party has the right to order smaller quantities, with the exception that not less than 60% of the contract value. 5. The order consists of 1 task. 6.The Ordering Party does not allow the submission of variant offers. 7.The Contractor guarantees that the products delivered to the Ordering Party will have a shelf life of at least 12 months from the date of delivery to the Ordering Party's registered office.
