The subject of this contract is the provision of services consisting in the preparation and delivery to the Contracting Authority of all-day meals prepared on the basis of the Contractor's kitchen for patients using 24-hour health services in the Provincial Independent Public Health Care Unit named after Prof. Eugeniusz Wilczkowski in Gostynin, hereinafter referred to as: WSZPZOZ Gostynin-Zalesie, Mental Health Centre – Branches: III C, V C, VI C and the Daily Psychiatric Ward, provided that the Daily Psychiatric Ward will use only lunches and nutritional supplements in accordance with the assumptions of the diet. 2. Within the scope of this contract, the Contractor undertakes to provide the Contracting Authority with a service consisting in the preparation and delivery to the Contracting Authority of all-day meals prepared on the basis of the Contractor’s kitchen for patients receiving 24-hour health services at the WSZPZOZ in Gostynin-Zalesie, Mental Health Centre – Branches: III C, V C, VI C and the Daily Psychiatric Branch, with the proviso that the Daily Psychiatric Branch will use only lunches and nutritional supplements in accordance with the assumptions of diets, assistance in unloading for trolleys, washing and disinfection of GN containers and thermoports on the Contractor's premises on its own, and the collection and disposal of nutritional waste, with due diligence, applicable law, health and safety regulations and sanitary and epidemiological regulations, in accordance with the contract, constituting Annex 2 to this PTS, which is an integral part thereof, the Contractor's tender form, constituting Annex 1 to this PTS / Annex 4 to the contract, which is an integral part thereof, and this PTS together with all content excluding Annexes 2-8. 3. The subject of this contract referred to in para. 1 of this Chapter covers the obligations of the Contractor which include: 1) przygotowanie i dostawy do oddziałów Zamawiającego, o których mowa w ust. 5 of this Chapter of meals taking into account the basic, easily digestible, low-fat, low-cholesterol, dairy-free, low-protein, vegetarian diet including 3 meals a day: breakfast, lunch and dinner and for diets: diabetic, with a limitation of substances stimulating the secretion of gastric, reducing, high-protein, low-fat - pancreatic, high-calorie, mushy juice taking into account 5 meals a day, i.e.: breakfast, lunch, dinner, 2nd breakfast, tea and additionally a night meal in the diabetic diet, 7 days a week twice a day (i.e.: breakfast and II breakfasts 7:30-8:15 and lunch, afternoon tea and dinner 12:45-13:30), in a thermos system, i.e. using specialized, certified thermoports, GN containers or equivalent, ensuring the maintenance of optimal temperature and quality of meals during transport from the Contractor's kitchen; 2) preparation and delivery to the Psychiatric Daily Branch of meals in the form of lunch, taking into account the diets referred to in paragraph 3(1) of this Chapter, for 5 working days a week, excluding holidays and public holidays, once a day (i.e.: lunch 12:45 - 13:30), in a thermos system, i.e. using specialized, certified thermoports, GN containers or equivalent, ensuring the maintenance of optimal temperature and quality of meals during transport from the Contractor's kitchen; 3) implementation of the subject of this order, referred to in this Chapter, in quantities resulting from the number of hospitalized patients of the Ordering Party, taking into account the requirements of the Ordering Party and dietary recommendations, with the proviso that the actual quantities of meals ordered by the Ordering Party will result from the current needs of the Ordering Party depending on the changing number of patients at individual WSZPZOZ branches in Gostynin - Zalesie, Mental Health Center - Branches: III C, V C, VI C and the Psychiatric Daily Branch, subject to Section 4 of the contract, which is Annex 2 to that FTZ; 4) performance of this contract, with the frequency according to the following schedule: a) breakfast (2nd breakfast) in hours. 7:30-8:15 (2nd breakfast delivered with breakfast); b) lunch (afternoon), dinner 12:45-13:30 (afternoon, dinner and night meal delivered with lunch) 5) preparation of meals based on your own kitchen; 6) employment of a specialist in nutrition and dietetics; 7) using only first-quality products for the production of dishes and serving meals from current production, in accordance with the principles of the Hazard Analysis and Critical Control Points HACCP system and allowing the Ordering Party the right to inspect the records in this respect and quality control of products, as well as access to documents identifying purchases and suppliers, with the proviso that the minimum daily cost of products necessary to prepare meals confirmed by purchase evidence should be a minimum of 51% of the price of the entire daily meal; 8) ensuring the required temperature of delivered meals to the Ordering Party, i.e.: a) breakfast: zupa (750 C), napój (800 C); Lunch: soup (750 C), II dish 630 C, land/mask/ groats 630 C, salads/sugars (40 C); Dinner: hot dishes 630 C; 9) delivery of meals referred to in this Chapter, at the latest within two hours of their preparation, prepared in accordance with the previously agreed menu, weight and calorific value, with the proviso that the Contractor will make these arrangements with the Employer's dietitian; 10) ensuring the correct weight of ready-made dishes, in accordance with Annex No. 2 to the contract, which is an integral part of this FTZ or according to arrangements with the Ordering Party's nutritionist; 11) providing meals to WSZPZOZ patients in Gostynin-Zalesie based on a 14-day menu agreed between the Contractor and the Ordering Party, broken down into individual diets and observing the rules of nutrition
