Description

1) The subject of the contract is the provision of occupational medicine services for police officers and employees of the District Police Headquarters in Chrzanów, Dąbrowa Tarnowska, Nowy Targ, Olkusz, Oświęcim, Sucha Beskidzka, Wadowice, Zakopane and the Central Bureau of Investigation of the Police, the Bureau of Internal Affairs of the Police and the Central Bureau for Combating Cybercrime performing official duties within the area of operation of the above-mentioned Police unit, as well as in the event of such a necessity, police officers and employees of other units of the Police of Małopolska Province - depending on the selected task. 2) The contracting authority divided the subject of the contract into 8 tasks: a) Task No 1 – provision of occupational medicine services for police officers and employees of the Małopolskie Police: KPP in Chrzanów; b) Task No 2 – provision of occupational medicine services for police officers and employees of the Małopolskie Police: KPP in Dąbrowa Tarnowska; c) Task 3 – provision of occupational medicine services for police officers and employees of the Małopolskie Police: KPP in Nowy Targ; d) Task 4 – provision of occupational medicine services for police officers and employees of the Małopolskie Police: KPP in Olkusz; e) Task No 5 – provision of occupational medicine services for police officers and employees of the Małopolskie Police: KPP in Oświęcim; f) Task No 6 – provision of occupational medicine services for police officers and employees of the Małopolskie Police: KPP in Sucha Beskidzka; g) Task No 7 – provision of occupational medicine services for police officers and employees of the Małopolskie Police: KPP in Wadowice; h) Task No 8 – provision of occupational medicine services for police officers and employees of the Małopolskie Police: KPP in Zakopane. 3) The Ordering Party requires the provision of services in the city: a) Chrzanów for task No 1 (KPP in Chrzanów), b) Dąbrowa Tarnowska for task No 2 (KPP in Dąbrowa Tarnowska), c) Nowy Targ for task No 3 (KPP in Nowy Targ), d) Olkusz for task No 4 (KPP in Olkusz), e) Oświęcim for task No 5 (KPP in Oświęcim) f) Sucha Beskidzka for task No 6 (KPP in Sucha Beskidzka), g) Wadowice for task No 7 (KPP in Wadowice), h) Zakopane or Nowy Targ or Waksmund or Czarny Dunajec for task No 8 (KPP in Zakopane). 4) The detailed description of the subject of the contract and the expected amounts of services are included in Annex 2 to the SWZ – price form and Annex 4 – draft contract. 5) The service will be provided depending on the needs of the Ordering Party. The anticipated amounts of medical services listed in Annex 2 to the CVT (price form) are forecast values. These values are indicated only for the purpose of selecting the most advantageous offer by the Ordering Party and do not constitute an obligation of the Ordering Party towards the Contractor. At the same time, the Ordering Party guarantees the performance of the contract at the level of at least 70% of the contract value specified in §7 sec. 2. 6) The Contractor guarantees that all tests will be carried out within a maximum of 3 working days, taking into account the day on which the police officer/employee reports for the tests. 7) A police officer/employee applying for control tests must have them carried out together with the decision issued on the day of application - provided that they apply by 12:00. 8) The Customer does not require that the tests be carried out within 3 working days counted one after the other. The Ordering Party requires that the performance of all tests does not exceed the maximum period of 3 working days, with the possibility of prior personal or telephone registration. Indication of the date of the tests in this case must be included within 14 working days of notification and the duration of the tests must not exceed the required 3 working days. 9) After selecting the most advantageous offer, and before the conclusion of the contract, the Contractor will present: a) the schedule of availability of medical staff and technical potential after the selection of the most advantageous offer and before the conclusion of the contract. The schedule must comply with the conditions for participation in the procedure; b) unit prices of tests included in study packages (in tables number 1-3, referred to in §3 sec. 2 of the model contract). 10) The contract will be concluded for a period of 12 months from the date of conclusion, but not longer than until the amount for which the contract will be concluded is exhausted.
