Description

The subject of the contract is the purchase and delivery of elements for osteosynthesis in the assortment and quantities given in the assortment and price forms constituting Annex 1A to the Specifications of the Terms of Order (SWZ).
The subject-matter of the contract was divided into 9 lots – Packages.
A detailed description of the subject of the contract is contained in Annex 1 to the SWZ.
The Ordering Party allows submission of partial tenders for any selected packages. However, the Ordering Party reserves the right to submit offers by the Contractor for the entire assortment in a given package.
The subject of the contract presented in the tender should have documents allowing its use in health care in the Republic of Poland in accordance with the requirements set out in the Act of 20 May 2010 on medical devices. The documents referred to above will be made available to the Ordering Party at each request.
The Contractor within the scope of Package No. 1, 2, 3, 6, 8, 9 is obliged to create an implant bank with the Ordering Party.
