The subject of the contract is the supply of thermoplastic blanks (Group 1), gel boluses and accessories for the planning of treatment and stabilization of the patient in the area of the head, neck, chest, upper limbs, pelvis and lower limbs together with mattresses for operating tables (Group 2) and stereotactic thermoplastic blanks for the BrainLab system (Group 3), in accordance with the requirements set out in the price forms constituting Annex 3, the model contract constituting Annex 4 and the limit conditions constituting Annexes 9A, 9B and 9C to the tender specifications.
