Description

1. Subject matter of the contract. The subject of the contract are successive deliveries of various medical devices, including specialized single-use sterile equipment, which have been divided into 4 (four) Parts described in the following table: PART NO. PART DESCRIPTION OF THE PART - SUBJECT OF THE ORDER 1 Central injections and other vascular accesses 2 Insulin syringes with fused needle 3 Insulin syringes with superimposed needle 4 Tuberculin syringes 2. The Ordering Party requires that the offered medical devices are allowed to be marketed in the Republic of Poland in accordance with the Medical Devices Act. 3. Detailed conditions for the performance of the subject-matter of the contract are described in detail in the Assortment and Price Form and the model contract for the award of the contract, which are annexes to the FTZ.
