Description

1.Proceedings are carried out in accordance with the Medical Activity Act of 15 April 2011 (Journal of Laws 2018, 160, i.e.) and the Act of 27.8.2004 on health care services financed from public funds (Journal of Laws 2017/1938, i.e.). 2.The object of the call for tenders is the provision of health services consisting of carrying out and describing studies in the field of computed tomography of patients at the Stw Hospital. Jadwigi Śląska in Trzebnica for a period of 24 months. 3.The detailed scope of the tests is set out in Annex 2 to the SWKO – in the Product and Price Form, consisting of two parts: ‘A’ and ‘B’. 4.The place where health services are to be provided will be rented by the Host Party from the Awarding Authority, a room located in the Chirurgia Building at level -1, located in the Stw Hospital. Jadwigi Śląska in Trzebnica. A separate lease agreement will be concluded with the Host Party in accordance with the model set out in the Annex. 4 to the SVKO
