Description

1. The subject of the order is the delivery of an automatic, high-throughput confocal microscope with automatic analysis of high content screening images together with control and software. 2. A detailed description of the subject-matter of the contract together with the requirements of the Contracting Authority with regard to the subject-matter of the contract is set out in Annex 2 to the SWZ – Description of the subject-matter of the contract and in Annex 3 to the SWZ – model contract. 3. Common Procurement Vocabulary (CPV) name and code: 38515000-8 Fluorescent and polarizing microscopes. The deadline for completing the subject of the contract is 8 weeks, counting from the date of conclusion of the contract. Conditions for participation in the proceedings: ‒ The Contractor has delivered at least one confocal microscope worth at least PLN 1 800 000 during the last three years before the deadline for submission of tenders and, if the period of operation is shorter, during that period. The subjective means of proof required of the economic operator shall include: 1) declaration of the contractor, within the scope of Art. 1 point 5 of the Public Procurement Law, on the lack of belonging to the same capital group within the meaning of the Act of 16 February 2007 on competition and consumer protection, with another contractor who submitted a separate offer, partial offer or request to participate in the procedure, or a declaration of belonging to the same capital group together with documents or information confirming the preparation of the offer, partial offer or request to participate in the procedure independently of another contractor belonging to the same capital group (template of declaration is attached as Annex 5 to the Public Procurement Law), 2) information from the National Criminal Register regarding the grounds for exclusion referred to in Article 108(1) points 1, 2 and 4 of the Public Procurement Law, drawn up not earlier than 6 months before its submission, 3) statement of the Contractor on the validity of the information contained in the declaration referred to in Article 125 paragraph. 1 of the Act, with regard to the grounds for exclusion indicated in Art. 1 point 3-6 of the PHP Act – the model declaration is attached as Annex 6 to the SWZ, 4) a list of deliveries made in the last 3 years, and if the period of activity is shorter in that period, together with their value, subject matter, dates of performance and entities to which deliveries were made and attaching evidence specifying whether those deliveries were properly made, the evidence in question being references or other documents drawn up by the entity to which the deliveries were made – (model Annex 9 to the SWZ). If the Contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of the document referred to in point. 7.6(2), lodges a document or documents issued in the country in which the economic operator has its registered office, domicile or domicile of the person to whom the information relates, or a document confirming, respectively, that its liquidation has not been opened, its assets have not been declared bankrupt, its assets are not being administered by a liquidator or a court, it has not entered into an arrangement with creditors, its business activities are not suspended or it is not in any other similar situation arising from a similar procedure provided for in the law of the place where the procedure is opened . The document referred to above should be issued no earlier than 6 months before their submission. The Contracting Authority shall not request the submission of personal evidence if: 1) it may obtain them using free and publicly available databases, in particular public registers within the meaning of the Act of 17 February 2005 on computerization of the activities of entities performing public tasks, provided that the contractor has indicated in the declaration referred to in art. 1 of the Public Procurement Law, data enabling access to these funds; 2) subjective means of proof is a statement, the content of which corresponds to the scope of the statement referred to in Art. 1 PZP. The contractor is not obliged to submit personal means of proof that the contracting authority has, if the contractor indicates these means and confirms their correctness and timeliness. In the scope not regulated by the Public Procurement Law or this SWZ, the provisions of the Regulation of the Minister for Labour Development and Technology of 23 December 2020 on subjective means of evidence and other documents or declarations that the contracting authority may request from the contractor and the Regulation of the Prime Minister of 30 December 2020 on the method of preparing and transmitting information and technical requirements for electronic documents and means of electronic communication in public procurement or competition procedures shall apply to declarations and documents submitted by the Contractor in the procedure. The Ordering Party, in accordance with the Regulation of the Prime Minister of 31 December 2020 on the method of preparing and transmitting information and technical requirements for electronic documents and means of electronic communication in public procurement procedures or competitions, specifies the necessary hardware and application requirements enabling work on Platformzakupowa.pl, i.e.: 1) permanent access to the Internet with a guaranteed bandwidth of not less than 512 kbps, 2) a PC or MAC computer with the following configuration: RAM min. 2 GB Ram, Intel IV 2 GHZ processor or its newer version, one of the operating systems - MS Windows 7, Mac Os x 10 4, Linux, or their newer versions, 3) installed any web browser, in the case of Internet Explorer minimum version 10.0, 4) enabled JavaScript, 5) installed Adobe Acrobat Reader or another program supporting the .pdf file format, 6) Encryption on platformzakupowa.pl is carried out using the TLS 1.3 protocol. 7) The time of data receipt by the purchasing platform is the date and the exact time (hh:mm:ss) generated according to the local time of the server synchronized with the clock of the Central Office of Measures. 8) Text automatically translated in your browsing language Automatically translated
