1. The subject of the contract is the service of transporting patients moving independently to / from the Dialysis Station. 2. At the latest before the conclusion of the contract, the Contractor shall provide the Ordering Party with a statement on compliance with the requirement referred to in paragraph. 5, i.e. that the person/persons performing activities related to the performance of the subject of the contract will be employed under an employment contract within the meaning of the Labour Code Act of 26 June 1974. 7. At each stage of performance of the contract, the contracting authority has the right to verify the contractor's/subcontractor's statement (concerning the form of employment of the persons referred to in para. 5). To that end, it is entitled, in particular, to request the competent labour inspectorate to carry out an inspection. 8. The Contracting Authority does not reserve the possibility of applying for the contract only by the Contractors referred to in Article 94 of the Public Procurement Act. 9. The Contracting Authority does not impose the requirements referred to in Article 96 of the Public Procurement Act. 10. The Contractor is obliged to have voluntary third party liability insurance of the value indicated in the draft contract. At the latest before the conclusion of the contract, the Contractor shall provide the Contracting Authority with documents confirming compliance with this requirement.
