Description

3.1. The subject of the contract is the successive supply of medicines of different 36 groups
3.2. CPV Common Procurement Vocabulary code: 33.60.00.00-6 Pharmaceuticals
3.3. Detailed description of the subject-matter of the contract is included in the price form – Annex 3/1 – 3/36
3.4. The minimum payment period required is 60 days.
3.5. If the medicine being procured is in the NFZ Active Substances Catalogue, the price of this medicine must not be higher than that indicated in the Catalogue.
3.6. The required period of non-variability of the net unit prices indicated in the tender shall be 6 months.
3.7. Medicinal products which are the subject of a contract shall be understood as medicinal products within the meaning of the Pharmaceutical Law of 6.9.2001 (consolidated text: OJ Of Laws 2008, No 45, item 271 as amended). The medicinal products offered must be authorised under the conditions laid down in Article 3 or 4 or 4a of the Pharmaceutical Law.
3.8. The contractor must indicate in its tender the share of the contract which it will subcontract.
4. The contracting authority allows the submission of partial tenders – 36 groups:
Group 1 – Aciclovir inj. 250 mg x 5.
Group 2 – Ambroxol amp. 0.015 g/2 ml x10.
Group 3 – Amiodarone inj. 50 mg/ml- 3 ml x 5.
Group 4 – Budesonide, Zaw do inh 0.125 mg/ml and 2 ml x 20, Zaw to inh 0.250 mg/ml and 2 ml x 20 Zaw to inh 0.5 mg/ml and 2 ml x 20.
Group 5 – Chlorpromazine * hydrochloride inj. i.m. 25 mg/5 ml x 5, inj. i.v. 50 mg/2 ml x10.
Group 6 – Ciclosporine inj. 50 mg/1 ml x 10 x 5.
Group 7 – Cocarboxylase inj. 50 mg/2 ml x 5 + RS.
Group 8 – Dexstranum 10 % r-r.p. 40 thousand (infusion, i.v.) F. 250 ml, 10 % r-r of dextran m. 40 thousand (infusion, i.v.) F. 500 ml.

Group 35 – Thrombin inj. 5 000 m + Reg. x2 (to oral stack).
Group 36 – Thrombin inj. 400 IU+ Reg. x 5 (to local application).
5. The contracting authority shall not provide for the award of complementary contracts referred to in Article 67(1). 1 pt. 6 and 7.
6. The contracting authority shall not allow variants to be submitted.
7. The time limit for performance of the contract.
7.1. Deliveries will be carried out successively for a period of 12 months from the date of signature of the contract, in accordance with the written orders of an employee of the hospital pharmacy.
7.2. A time limit for performance of the contract not exceeding two working days from the date on which the order is placed by fax.
7.3. The possibility of urgent delivery within the shortest possible time agreed with the Head of Hospital Pharmacists, but not more than 24 hours from the date on which the order was placed by telephone, confirmed by fax.
7.4. Delivery, together with the lodging and unloading of the goods, will take place at the storehouse of the Contracting Authority. The goods will be collected by a person authorised by the Head of Hospital Pharmacists. At the time of receipt of the goods, the employee will be required to examine whether the delivery is in accordance with the attached documents and the contract in quantitative and qualitative terms. The examination shall include the recalculation of the quantity of packaging and the determination of their condition and, in the event of damage to the packaging, verification of the condition of its contents.
7.5. The goods will be delivered to the Contracting Authority in accordance with the conditions laid down in the Regulation of the Minister for Health of 26.7.2002 on the procedures for good distribution practice (Journal of Laws Journal of Laws 2002, No 144, item 1216).
7.6. In the event of performance of the contract relating to transport by subcontractor, the Contractor shall be liable for the subcontractor’s acts, omissions and negligence, as for his own actions, omissions and negligence, including compliance by the subcontractor with the requirement set out in point 7.5.
