The KKH has concluded a contract with Oska Health Medical GmbH in accordance with § 140a SGB V. The purpose of this contract is to provide renal insufficiency patients with a digital program specifically designed for the treatment and management of patients with renal insufficiency. It is carried out in close cooperation with the attending physicians, who are responsible for therapy planning and adaptation of treatment. In addition, a coaching approach is integrated to support patients in their daily health management. Coaching includes advice and motivation on healthy lifestyle habits, the correct application of therapy recommendations and mental support to strengthen the self-efficacy of patients. The aim is to improve patients’ quality of life, slow the progression of the disease and minimise complications through holistic care, consisting of medical expertise and personal coaching. The KKH will conclude this contract in the same terms with any other market participant in the European internal market who has the following selection criteria: The market participant must be a medical device manufacturer in accordance with § 140a(3)(6) SGB V in the field of the care of patients with renal insufficiency and a management company within the meaning of § 140a(2) SGB V. Evidence: • Business documents from which the status as a medical device manufacturer for the care of patients with renal insufficiency can be inferred • References on cooperation with other health insurance companies as a management company The market participant must prove the qualification of the medical specialists used for coaching. The following qualifications must be met by medical professionals: The coaches must have a sound education and experience in the care of renal insufficiency patients, with the mandatory use of specialists from areas such as nephrology, nutritional counseling, diabetology, cardiology or comparable medical disciplines. Evidence: • Evidence of the qualifications of the health advisers employed, such as diplomas or certificates in relevant medical disciplines . • Evidence of the experience of health counsellors in caring for CKD patients, e.g. through testimonials, certificates or references. • Documentation of training and further education measures that ensure the continuous qualification of the coaches. • Evidence of the use of coaching programmes in practice to support CKD patients, including concrete success reports or references. • Proof of compliance with quality standards in the implementation of the coaching measures, e.g. through the application of established concepts or guidelines on disease management and health promotion. The conclusion of a contract on equal terms is carried out by means of a ‘declaration of accession’ by the operator. This must have been received by the KKH no later than 31.01.2025 at 12:00 CEST. Thereafter, there is no longer any right to conclude a contract with the same content.
