1. The subject of the contract is the purchase, delivery and assembly of furniture equipment for the ZUS Branch in Bielsko-Biała and the ZUS Inspectorate in Żywiec. 2. Place of delivery: 1) ZUS Branch in Bielsko-Biała at ul. Krasiński 34, 36. 2) Inspectorate in Żywiec at ul. Gingerbread 2. 3. Delivery, unloading, furniture should be complete (in full), ready for use in accordance with their intended purpose and bringing furniture at their own expense and risk to the Ordering Party's premises. 4. Self-assessment and collection of packaging at your own expense. 5. One-time delivery of the assortment in full, divided into the following locations: 6. ZUS Inspectorate in Żywiec at ul. Gingerbread 2: 1) a simple 140 x 80 desk with manual height adjustment - 100 pcs; 2) mobile desktop container - 100 pcs; 3) mobile container - 80 pcs; 4) file cabinet - 100 pieces; 5) wardrobe - 40 pcs.; 6) economic cabinet - 35 pcs.; 7) stationary rack - 70 pcs.; 8) mobile container for printer - 25 pcs.; 9) Straight table - 10 pcs.; 10) ISO conference chair - 50 pcs; 11) sheet metal cabinet - 5 pcs. 7. ZUS Branch in Bielsko-Biała at ul. Krasiński 34, 36: 1) a simple 140 x 80 desk with manual height adjustment – 20 pieces; 2) mobile desktop container - 20 pieces; 3) file cabinet - 50 pieces; 4) metal actuator cabinet - 7 pcs; 5) swivel chair with headrest - 10 pcs. 8. A detailed description of the subject of the contract (OPZ) is attached as Annex 1 to the draft provisions of the contract, which are attached as Annex 2 to the FTZ.
