Description

The subject of the contract under § 140a SGB V is a telemonitoring of insured persons spread over three lots in the indications heart failure, COPD and diabetes mellitus type 2. Telemonitoring is complemented by accompanying coaching, which gives insured persons the opportunity to reflect on and optimise their own health behaviour. Through regular counselling and training - including both medical and psychosocial aspects - the competence of patients in dealing with their disease is to be strengthened. This helps to increase their quality of life, as they can not only be better informed and motivated, but also have a more self-determined influence on the course of the disease.
