Description

1. The subject of the contract is the supply of medical equipment as part of the implementation of the investment task entitled: ‘Supply of medical equipment’. Purchase co-financed by the state budget from Part 46 - Health. 2. The subject-matter of the contract has been divided into individual lots: Task 1. An anaesthetic device. Task 2. Neurophysiological monitoring system. 3. The Contractor shall deliver the subject of the contract to the seat of the Ordering Party, at his own expense, also bearing the cost of loading and unloading and insurance during transport, will make the first commissioning, in accordance with the requirements set out in Annex 2 to the Contract. 4. A detailed description of the subject-matter of the contract is set out in Annex 1 to the Agreement – Assortment and price form and Annex 2 to the Agreement – Technical requirements – boundary conditions. 5. The Ordering Party does not allow the delivery of remanufactured equipment, used or being the subject of demonstrations, demonstrations, etc. The offered subject of the contract must be brand new and approved for marketing and use, in accordance with the Act of 7 April 2022 on medical devices (Journal of Laws 2022, item. 974 as amended), bear the CE mark to confirm that it meets all relevant legal requirements in force in the EU, including requirements regarding safety, health protection or the environment. 6. The Contractor shall transmit the subject-matter of the contract to the Contracting Authority by means of a handover document, i.e. a delivery and acceptance report. Together with the delivery and acceptance protocol, the Contractor will provide relevant documents allowing the subject of the contract to be used, the manufacturer's documentation, user instructions and a guarantee for the delivered subject of the contract.
