Description

1. The subject of the contract is the successive delivery of implantable medical devices. 2. The subject of the contract for individual parts of the contract is described in detail in Annex 2 to the SWZ. 3. The contracting authority shall allow partial tenders to be submitted. 4. The order has been divided into three parts (packages). 5. The contracting authority shall allow tenders to be submitted for one, several or all lots. 6. The scope of the contract covers the supply of medicinal products and single-use medical devices. 7. At each request of the Employer, the Contractor is obliged to provide documents confirming that the medicinal products offered by the Contractor in terms of packaging, marking and marketing comply with the requirements of the Pharmaceutical Law Act of 6 September 2001, and the offered medical devices are placed on the market and used in Poland in accordance with the provisions of the Act of 7 April 2022 on medical devices. 8. The conditions for the implementation of the Subject of the Order are also included in the Draft Provisions of the Agreement, constituting Appendix No. 8 to the SWZ.
